If you desire to be a judge of election for Provo City in the upcoming municipal elections to be held October 4 and November 8, 2005, please see the application form.

To be a judge, attendance at a judge training is required. You will receive $15 for attending. Training sessions have been scheduled on Tuesday, September 27 from 5:00 - 6:30 p.m and Thursday, September 29 from 10:00 - 11:30 a.m. in the Council Chambers, 351 West Center, Provo.

You must fill out a W-9 form to be paid for serving as a judge. Judges are paid $80 per election day.

If you are able to attend the training and serve as a judge for the Primary and/or General election, please fill out the form and return it to the Office of the City Recorder, PO Box 1849, Provo, UT 84603 by August 17.
