Bicycle licenses are valid through the life of the bike with the same owner. To register a bicycle appropriate fee and the registration form must be submitted to the Provo City Licensing Administrator at 351 West Center, Provo, Utah. The registration form includes:

Other Identifying Marks
The manufacturer’s serial number is usually on the underside of the bike frame. If this number has been obliterated or does not exist, the police department has a metal stamp kit and can stamp a number on the bike.
