Closure


Upon closure of your Provo business license, please notify the business licensing department.  The notification needs to include the business name, location and last date of business activity.  Please notify the Provo City business license department using one of the following methods:

Email the business name, license number, address and date of closure to licensing@provo.utah.gov.
 
Complete the “Business Closure” section on the business license renewal form and return it to the Business License department.
Send a letter which includes the business name, license number, address and date of closure to:

If our office is not notified that a business has closed, we will continue to mail renewal notices and assess a renewal fee.  If the renewal fee is not received in our office by January 31st, a $25.00 late fee will be assessed.
