Rental Dwelling
If you are applying for a Rental Dwelling License you should review the Frequently Asked Questions so you will know what information is required to complete the application. If you are not able to use the online application, a PDF version of the application is available. If you use the PDF version, you will need to print out the document and mail it in along with your payment to Provo City Licensing, P.O. Box 1849, Provo, UT 84603.
