Request Your Utah Criminal History
The Provo Police Department is able to provide you with a copy of your Utah criminal history. The following will be necessary to fill your request:
- Legal identification
- A Utah criminal history request form
- $10.00 fee
If you are in need of your criminal history from another state, you will need to contact your local FBI office. Information is available from their website.
If you are in need of your juvenile criminal history, please contact the juvenile court.
