Utah State Senate BIll 81, passed March 24, 2017, certain home based business ar no longer to be charged a license fee unless the combined offsite impact of the home based business and the primary residential use materially exceeds the offsite impact of the primary residential use alone.
Before submitting an application, please review the Provo City Code
Chapters 14.41.050 for Minor and 14.41.060 for Major Home Occupations.
If it is determined that you will be required to obtain a Home Based Business License there is a $37 fee.
To apply for a Provo City Business License you will need to fill out an application to be submitted online.
The following information will be required:
Business Name and Physical Location
Main business phone number and mailing address
Business Ownership Type (Corporation, Partnership, Proprietorship, LLC)
Number of Employees at location
Projected Opening Date and expected Business Hours
Nature of Business and Detailed description of business being conducted
Three (3) Emergency Contacts
Property Owner information
Federal Employee ID Number or Federal Tax ID Number
There may be additional information required, depending on the type of business you are applying for. This includes:
Type of Business (Home Based)
Kind of Business (Non-profit, Taxi, Restaurant, Mobile Food Truck, Towing, etc.)
State License Number and License Type
Permanent or Temporary Sales Tax Number
List of Corporate Officers and main business contact information
Copy of Professional/Occupational License
Industrial Wastewater Pretreatment Questionnaire
Food Service Establishment Questionnaire
All applications are required to submit proof of active registration with the Utah Department of Commerce and a photocopy of the applicant's Picture ID.

Do I Need A Business License?
If you are operating in a commercial location within Provo City limits you are required to apply for and maintain an active Provo City Business License.
You are also required to apply for and maintain an active Provo City Business License if your business includes the following:
If you are keeping, conducting, or maintaining a mobile home park
If you are engaged in distributing tangible personal property at a retail or wholesale
If you are engaged in distributing goods, or providing services to others for payment or reward, including:
some day care providers
restaurants and food trucks
any persons engaged in trades and crafts
professionals, including doctors, lawyers, accountants and dentists
banks, savings and loan associations
real estate agents

Why Should I Get a License?
Most importantly, you should obtain a business license before conducting any business in Provo because a valid license is required by local and state law. States and municipalities require businesses to be licensed in order to protect the consumer, the community, and the business owner. The licensing process aims to help the business owner comply with local and state regulations, and to make sure the business is operating safely. For example, the fire department ensures safe conditions such as having enough emergency exits and fire extinguishers, and public works will inspect businesses to make sure contaminants aren't released into our public water.
