Business Licensing Requirements



All businesses are required by law to obtain a valid Provo City business license before opening or beginning operations.  Not sure if you need a license? Call the business licensing department. The processing time for a business license is approximately 14 working days from the time your application is submitted and fees paid. This time frame may vary depending on approvals, inspections, and other steps required. Before submitting a business license application to Provo City, please review the information below, as well as FAQ's and Before You Apply.  A business license cannot be issued until all state and local requirements are met, and documentation provided. Please be aware Provo City does not issue temporary business licenses. If you are operating your business from more than one location you will need a separate application and license for each location. You must also bring a valid government issued identification with you to Business Licensing when you turn in your application and pay your fees. Acceptable forms of identification include: State driver’s license; State I.D. card; U.S. passport; or Military I.D.



Your Application

Please review the business license application here.  You will need to provide the following information on the application:
  • Business name
  • Physical street location and phone number of business
  • Mailing address if different than physical location
  • Owner/partner(s) name, home address, phone, date of birth
  • List of corporate officers, if applicable
  • Type of business structure (partnership, proprietorship, LLC, etc.)
  • After hours emergency contact name and phone number
  • Projected opening date of business
  • Federal Identification Number (EIN)
  • Sales Tax number (specific to Provo location)
  • Copy of Professional/occupational license if applicable
  • Registration with Utah Department of Commerce
  • Number of employees


Additional Requirements

Businesses which require a separate application or have additional requirements are

Contact the Health Department at (801) 851-7525 about an inspection if your business:

  • Sells, handles or prepares food, including restaurants, gas stations, concession stands, snow cones, food trucks,   etc. ( Must also contact Dept. of Agriculture (801) 538-7124) 
  • Produces body art (permanent make up and tattoos)
  • Has a tanning bed
  • Has any size or type of pool
  • Is a daycare or school which prepares food
  • Handles alcohol
